Physician Referral
We want to continue providing excellent health care in your patient’s home. The process to begin care with us is fast and easy. Simply fill out one of our referral forms and fax it to our intake department. Please remember to include a H&P and notes from the patient’s last visit. If you have any questions, feel free to call and speak with our intake department or your Wilshire Account Executive, all of whom are happy to assist in any way to make the process efficient for you. If you are looking for assistance in your recovery or have a family member who would benefit from having skilled help in the home – reach out to Wilshire!
While Hospice does have to be “ordered” by your primary care physician, our Intake team is happy to reach out to them on your behalf to discuss your care and receive orders. Once your physician approves care, one of our nurses will be in touch with you to schedule your first visit.
